Terms & Conditions
There is a program fee of $120 per team or $20 per individual. Payment can be made at Coach T.B. Thomas Sports Center office, 701 W Washington St, weekdays, 8 a.m. to 8 p.m. Acceptable payments methods include credit card, cash, or checks made payable to City of Hartsville.
Activities on City of Hartsville (City) property shall not discriminate based on race, religion, color, national origin, sex, disability or age.
STATE OF SOUTH CAROLINA} RELEASE OF LIABILITY AND ASSUMPTION OF RISK
CITY OF HARTSVILLE}
I, the undersigned, on behalf of myself and/or my child, hereby voluntarily release, discharge, waive and relinquish any and all claims or causes of action for personal injury, property damage, or wrongful death which may arise out of or in connection with my participation and/or my child’s participation in the following sport: Kickball, regardless of how such injuries or damages may occur.
I understand and acknowledge the above-mentioned sport activities have known and unanticipated risks that could result in physical or emotional injury, paralysis, death, or damage to me, to my child, to property or to third parties which no amount of care, caution, instruction, or expertise can eliminate. In consideration of me being allowed and/or my child, being allowed to participate in the activity listed above; I, on behalf of myself and/or my child, expressly and voluntarily assume all risk of injury or death, whether foreseeable or not, sustained in connection with my participation, or my child’s participation, on or around these events to included transportation to and from such activities.
Accordingly, under no circumstances will I, nor any of my heirs, distributes, guardians, legal representatives or assigns present or bring any claim for personal injury, property damage or wrongful death against City of Hartsville and/or any officer, director, member, agent, servant or employee of the City. Further, I agree to indemnify and hold City of Hartsville harmless from liability for injury to persons or property, which may arise from my participation, and/or my child’s participation, on or around above-mentioned activities. I, for myself and on behalf of the minor participant listed below, also agree to a blanket event release of all rights related to me or to my child’s audio and photographic image that may arise out of my participation or my child’s participation in the above-mentioned.
This event release includes any and all marketing, promotion, or advertising specific to any event that may occur anywhere and anytime on any media as later used by City of Hartsville or any of its representatives or assigns. This Release shall be construed and enforced in accordance with the laws of the State of South Carolina. I understand this Release is severable and should any clause be found invalid, the rest of the Release shall remain in effect and be enforceable.
